DEPARTMENT OF PHARMACY

The Pharmacy Department of Hospital Sultan Abdul Aziz Shah (HSAAS) plays a vital role in providing comprehensive, safe, and high-quality pharmaceutical services to patients, healthcare staff, and the hospital community. These services include medication supply, clinical pharmacy care, support for specialized therapies such as cytotoxic preparations, parenteral nutrition, and galenical products, as well as drug information management. Supported by various specialized units that complement each other, the Pharmacy Department is committed to strengthening the healthcare system through rational, effective, and evidence-based use of medicines, in line with technological advancements and current practices.

Ambulatory Pharmacy Services involve two main units: the Outpatient Pharmacy (OPD) and the Non-Formulary Pharmacy (NF). OPD supplies medications to outpatients receiving follow-up treatments and provides flexible facilities such as the Pharm4u (Medication by Post) service and the 24-hour PharmaBox lockers. The NF Pharmacy supplies non-formulary medicines at affordable prices, specifically for prescriptions from HSAAS. This unit also collaborates with the Medical Social Work Department in managing medication supply through the JPA Medication Assistance Form, ensuring continuity of patient treatment.

Inpatient Pharmacy Services are responsible for supplying medicines through the unit dose and ward stock systems for warded patients, including FPP (Full Paying Patient) services. This service also covers Clinical Pharmacy, where clinical pharmacists conduct medication therapy reviews, provide counseling, monitor adverse drug reactions, and adjust treatment dosages. Therapeutic Drug Monitoring (TDM) is also carried out for certain drugs such as gentamicin, vancomycin, and phenytoin to ensure safe and optimal dosing. Direct clinical involvement makes this pharmacy service a cornerstone of effective, evidence-based medication use.

Pharmaceutical Production Services include the preparation of specialized medicines in compliance with strict quality standards, aseptic practices, and safety protocols through four dedicated units. The Total Parenteral Nutrition (TPN) Unit provides complete parenteral nutrition for patients unable to take food orally or enterally. The Galenical Unit produces specialized preparations such as creams, ointments, syrups, suppositories, and medicines not commercially available. The Cytotoxic Drug Reconstitution (CDR) Unit is responsible for the aseptic preparation of cytotoxic and anticancer drugs in specialized isolator rooms, while the Nuclear Pharmacy Unit manages the preparation of radiopharmaceuticals for diagnostic and therapeutic purposes, with strict compliance to radiation safety regulations.

The Medical Store Unit (MDS) and the Drug Information Services (DIS) Unit work together in managing specialized medicines at HSAAS, including import permit management, as well as formulary monitoring and updates. MDS oversees the overall inventory management of hospital medicines and medical supplies. Meanwhile, DIS serves as a drug information reference center, manages applications for the inclusion of new drugs into the formulary, coordinates the issuance of Adverse Drug Reaction (ADR) cards, and monitors medication safety issues.
