Service Profile: Occupational Medicine Clinic (OMC), HSAASIntroductionThe Occupational Medicine Clinic (OMC) at Hospital Sultan Abdul Aziz Shah (HSAAS) provides comprehensive occupational health services encompassing the prevention, monitoring, and management of occupational health risks among HSAAS staff. The services are specifically designed to identify and control employees' exposure to physical, chemical, biological, ergonomic, and psychosocial hazards in the workplace. The primary objective of the clinic is to ensure that the workforce remains healthy, safe, and productive while safeguarding the well-being of HSAAS personnel in accordance with the Occupational Safety and Health Act (Amendment 2022). Core Services1. Fitness Assessment & Occupational RehabilitationI. Pre-Employment / Pre-Placement Medical ScreeningMedical examinations conducted to ensure that individuals are in optimal health, free from medical conditions that may affect work performance, and to establish a baseline health profile prior to employment. II. Fitness for Work (FFW)A comprehensive medical assessment to determine an employee's fitness to perform assigned duties. The assessment considers the employee's current health status, job scope, and associated occupational risks. III. Return to Work (RTW) ProgrammeThis programme supports employees with disabilities or functional limitations in their transition back to work following recovery from serious illness or injury. 2. Occupational Clinical Disease ManagementI. Occupational Diseases and Occupational PoisoningManagement and investigation of cases directly resulting from workplace exposure. Cases are identified through referrals from specialist clinics within HSAAS, administrative referrals by Heads of Department when occupational diseases or poisoning are suspected, as well as findings from audits conducted by agencies such as the Department of Occupational Safety and Health (DOSH). II. Work-Related DiseasesMonitoring and management of pre-existing health conditions with multifactorial causes that may be aggravated by the work environment or occupational factors. 3. Medical SurveillanceMedical surveillance involves pre-employment, pre-placement, periodic, pre-transfer, and pre-retirement medical examinations for staff exposed to specific hazardous substances or occupational risk factors. These assessments are conducted to evaluate exposure levels and detect adverse health effects at an early stage. The programme complies with relevant legislation and regulations, with particular emphasis on employees exposed to hazardous chemicals (such as formaldehyde), ionising radiation, cytotoxic drugs, and other occupational hazards. 4. Medical Removal Protection (MRP)Medical Removal Protection (MRP) is a mandatory protective measure whereby employees who are confirmed or suspected to have occupational diseases or occupational poisoning are immediately removed from hazardous work areas. This measure is applied only to specific occupational hazards regulated under relevant legislation, such as excessive formaldehyde exposure or occupational noise exposure exceeding permissible limits. Affected employees may only resume work after complete clinical recovery, normal laboratory findings, and certification of fitness for duty by an Occupational Health Doctor. 5. Occupational Screening and Immunisation ProgrammesI. Tuberculosis (TB) ScreeningAdministration of the Mantoux test for pre-placement TB screening among HSAAS clinical staff, as well as testing for referrals from departments, sections, units, or wards within HSAAS. II. Occupational Immunisation ProgrammeTargeted vaccination programmes based on occupational risk assessment, including: A. Hepatitis B Provided for newly appointed staff and students of the Faculty of Medicine and Health Sciences (FMHS), Universiti Putra Malaysia, with an antibody titre below 10 mIU/ml. B. Typhoid Mandatory every three years for employees involved in food handling. C. Dengue and Other Vaccinations Administered in accordance with prevailing guidelines and operational requirements. 6. Occupational Disease Management & Emergency IncidentsI. Needlestick and Sharp Injury & Splash Injury ManagementA structured monitoring protocol is implemented for employees who sustain needlestick or sharp injuries. Following initial treatment by the Department of Medicine, affected staff are required to attend follow-up assessments at the OMC during the sixth week and third month to evaluate the risk of blood-borne infections. 7. Combined ClinicThe Combined Clinic is a multidisciplinary clinic involving collaboration between the Occupational Health Doctor and other healthcare professionals, including Occupational Therapists (OT), to provide comprehensive assessment and management of employees. Through this collaborative approach, employees are jointly evaluated in terms of their medical condition, work function, fitness for duty, job modification requirements, ergonomic considerations, rehabilitation, and return-to-work planning. Joint case discussions enable the development of more effective and holistic management plans.
